HOW TO SIGN UP FOR SERVICES
A member (client) is eligible if over age 18, is from Beltrami County or a contracting county, has a diagnosis
of serious and persistent mental illness, and the diagnostic assessment recommends long-term mental health services.
The diagnostic assessment can be completed through Hope House.
Any person –potential client or other source- may refer by calling 218-444-6748, x109.
